Our reablement service, which provides support in the home for people leaving hospital, has once again proved that it delivers top quality care to the people of Kingston.

The recent visit by the CQC confirmed that the service provides an outstandingly effective provision.

The CQC interviewed people using the service and reported them as saying: “"Very kind and helpful." Another person commented, "They were very polite. They were a help to me." A third person commented, "I'd like to say the help and care I get from the Enablers is second to none. Everyone is so helpful and caring and I appreciate it very much."

The reablement service identifies the support required following a discharge home from hospital. The team visits people in their homes and helps them to regain as much independence as they can after a period of ill health, injury or have long term health conditions that may impact on them losing their independence and/or having difficulties coping at home.

The team come into people’s homes to provide an initial assessment and if required reablement will be provided to help improve their ability to manage at home. They liaise with a wide range of other health and social care teams such as occupational therapy, district nurses and social workersto ensure that the needs of each person are met appropriately.

Claire Georgie, Reablement Team Manager, said: “We were so delighted to receive this endorsement of our service. We work hard to provide a joined-up service with other teams and Kingston Council to make sure we give the best and most appropriate care and support to people to maximise their independence.”
