Urgent Care & Support Service for Care Homes
The urgent care and support team assesses, diagnoses, prescribes and treats acute health problems, to prevent hospital admissions for patients living in care homes. Our team of paramedics and nurses offer 2-hour clinical triage and same day visits.
The team has the clinical knowledge and expertise to assess, diagnose and treat a wide variety of conditions including gastrointestinal, genitourinary, respiratory, neurological, ENT (ear, nose and throat), skin conditions and more.
The urgent care and support team is part of the Urgent Community Response service.
Our service
We aim to provide a prompt, evidenced-based, patient-centred and holistic service to care home residents in the Kingston borough. We signpost to other services where necessary.
Our service helps to prevent unnecessary A&E attendance/hospital admissions by bringing expert care directly to care home residents.

What we do
Our team treats a variety of conditions. They have expertise in dermatology (ash, cellulitis and wounds), gastrointestinal conditions (abdominal discomfort, nausea/vomiting, bowels), genitourinary conditions (catheter problems), respiratory conditions (asthma, COPD, infection, cough), ENT (ear, nose and throat), neurological issues (headaches), musculoskeletal conditions (back pain) circulatory problems (DVT, cardiac) and palliative care.

We are part of the Urgent Community Response service.
- We are available Monday – Sunday 8am – 8pm. Last referral accepted 6pm
- Out-of-hours support – care homes should contact NHS 111 *6
- Contact Your Healthcare at Home, Night Response service and Palliative & Catheter Care on 07436 531237. Available 10pm to 7am.

If we make any profit, it is reinvested into our community services

You can also call for emergency referrals to our Rapid Response service (2-hour urgent community response) or Urgent Care Support Service (care homes).
It is possible to self refer to some of our services (e.g. rapid response, children’s speech and language). You can also call our single point of access team on 020 8274 7088 for referrals to our Rapid Response service (2-hour urgent community response).
