Community nursing service provides skilled nursing care to adults aged over 18 years in their own homes, in residential homes or in a clinic setting. Patients that we provide nursing care to are primarily housebound and will be expected to attend their doctor’s surgery as they recover.

Community Nursing teams are attached to GP surgeries, working as part of the integrated team. They work closely with families, carers and other professionals including those in other health care settings, private and voluntary sectors.

Community nursing supports and encourages people with a disability or long term condition to live independent lives, providing a range of services and treatment to enable individuals to avoid unnecessary hospital admissions and to facilitate early discharge where hospital admission has been necessary. Community Nurses also provide care to patients requiring palliative and end of life care.

Community nursing teams work with our colleagues within Social Services from the Royal Borough of Kingston ensuring effective working relationships and good communication in relation to patient care.

Our community nursing teams cover practices across the borough in New Malden, Worcester Park, Kingston, Surbiton and Chessington, and are all based in Hollyfield House.

Community Nurse Teams are led by District Nurses. These are specialist practitioners in this area of work. Community Matrons are key members of the teams and they also provide expert care for patients with one or more long term conditions, for example, diabetes, Chronic Obstructive Pulmonary disease (COPD), Coronary Heart Disease, Parkinson’s disease, Multiple Sclerosis and HIV. Along with Community nurses and Healthcare Assistants the team provide nursing care 365 days a year.

Between 8am and 6pm Monday to Friday and 9am to 5pm Saturday and Sunday/Bank Holidays contact the Single Point Access 0n 020 8274 7088.

Outside these hours contact via 111 who will contact the Community Nursing Services. The Community Nurse team provides nursing care up to 12midnight. We work in partnership with the 8 to 8 Night Response Service to ensure that support is available to patients overnight.

Your Healthcare works in partnership with Princess Alice Hospice which provides overnight services to patients in their own homes. This service provides palliative and end of life care to patients, supporting them with advice and symptom control.  Patients and carers thought to be in need of this overnight support will have been provided contact details. The 8-8 Night Response Service provide urgent nursing care to prevent hospital admission overnight if appropriate. This service can be contacted via 111.
