NHS Continuing Health Care is the name given to a package of care which is funded solely by the NHS for adults outside of hospital who have ongoing health care needs and who are assessed as meeting the criteria under the National Framework. Continuing health care can be delivered in any setting including your own home, Nursing home or care home.
NHS Funded Nursing Care is funding provided by the NHS to people in Nursing Homes and funds the nursing element of the care to ensure those people eligible receive appropriate care.
The Continuing Care/Funded Nursing Care Team, as part of Your Healthcare, carry out assessments in Nursing Homes and oversee assessments in other settings, working in partnership with other community agencies. They also act as coordinators to ensure cases are presented to health commissioners for agreement on funding.
People in Nursing Homes will automatically be assessed and reviewed by the team. Those living in other community settings who need advice on Continuing Care funding are advised to contact the District Nursing Teams or the Continuing Care/Funded Nursing Care Team direct.
