Your Healthcare provides support, advice and education for people with diabetes living in the Kingston area.
The service is led by a community diabetes specialist nurse whose main focus is the DESMOND programme, but she may visit patients at home or in a GP surgery (mainly to support GPs and community or district nurses).
DESMOND is a nationwide group education programme developed by experienced health care professionals and people who have diabetes.
The programme is for those newly diagnosed with Type 2 Diabetes and there is also Foundation Desmond for those with established diabetes.
Patients receive 6 hours of structured education in a group setting with a family member or friend.
DESMOND is delivered by healthcare professionals who are specially trained to deliver the course. Its aim is to support the person with diabetes, to become an expert in his/her condition.
At DESMOND you can:
- Talk about your concerns
- Get answers to your questions
- Discuss the options for looking after your diabetes
- Be supported in making your own choices about how you deal with your diabetes
- Find out about what support you can expect from your local health service
