Your Healthcare provides support, advice and education for people with diabetes living in the Kingston area.
The service is led by a Community Diabetes Specialist Nurse whose main focus is the DESMOND programme, but she may visit patients at home or in a GP surgery (mainly to support GPs and Community or District Nurses).
DESMOND is a nationwide structured group education programme developed by experienced health care professionals in participation with people who have diabetes.
The programme is for those newly diagnosed with Type 2 Diabetes and there is also Foundation Desmond for those with established diabetes.
Patients will receive 6 hours (9.15am – 4.30pm) of structured education in a group setting for up to 10 participants with a family member or friend.
It is delivered by two accredited healthcare professionals trained to deliver the course and aims to support the person with diabetes, to become the expert.
The course has been designed to meet NICE and Department of Health/ Diabetes UK quality standards and criteria for structured education.
Sessions , which are once a month, are currently between 9.15am -4.30pm at:
