The team consists of experienced nurse practitioners with advanced physical assessment skills. We assess, diagnose, prescribe and treat acute health problems, to prevent hospital admissions for patients living in care homes. We offer 2-hour clinical triage and same day visits.

Our team has the clinical knowledge and expertise to assess, diagnose and treat a wide variety of conditions such as:

  • Dermatology - rash, cellulitis and wounds.
  • Gastrointestinal - abdominal discomfort, nausea/vomiting, bowels.
  • Genitourinary - catheter problems/associated issues and UTI.
  • Respiratory - asthma, COPD, infection, cough.
  • Ear Nose and Throat
  • Neurological - headache, head injury, dizziness and syncope.
  • Musculoskeletal - back pain, reduced range of movement in limbs, arthritis associated problems and muscular pain.
  • Circulatory - DVT, cardiac.
  • Palliative Care - advanced care planning, hospital admission decision for reversible causes, lasting power of attorney input if the patient is unable to communicate wishes, end of life medication, liaison with Princess Alice Hospice, GPs and other healthcare professionals.

We aim to provide a prompt, evidenced-based, patient centred and holistic service to care home residents in the Kingston borough. We signpost to other services where necessary.


Our service provides:

  • Prevention of unnecessary A&E attendance/hospital admissions
  • Support for hospital discharges
  • Prescribing medications, all wound products and catheter supplies
  • Palliative care/assessments including prescribing anticipatory medication
  • Management of residents with complex care needs
  • Advanced care planning
  • Comprehensive management and care plans
  • Intravenous therapy support following discharge from acute hospital
  • Training and education where identified, including care home staff
  • Falls ‘pick up’ service

Urgent Care & Support Service is part of the Urgent Community Response service

  • We are available Monday – Sunday 08:00 – 20:00. Last referral accepted 18:00
  • Out of hours support - care homes to contact NHS 111 *6
  • Contact Your Healthcare at Home, Night Response service and Palliative & Catheter Care on 07436 531237. Available 10pm to 7am.

We accept the following referrals:

  • Direct from all nursing homes in the Kingston borough
  • All Healthcare professionals and services
  • Residential and learning disability care homes via GPs but direct referrals accepted for the Falls ‘pick up’ service
