Your Healthcare shares information about the care you have received to help health and social professionals to understand the needs to service users and the quality of the treatment and care provided and reduce inequalities in the care provided.
You may receive care and treatment from a number of places such as your GP practice, Hospitals and Community Services. By bringing this information together from all the different places as part of an integrated approach, we can quickly and efficiently compare the care provided in one area with the care provided in another, so we are able to see what works best and where.

Consent forms are essential to enable us to share your information confidentiality, appropriately and proactively, in order to create better links between services who are involved in providing care for you.
If you, or someone you are working with or supporting, wants to request access to health & care records or reports please submit your request using our new secure portal by following the three steps below:
Login and submit a new request on the portal, ensuring you have the required documents before doing so – a list of documents we accept can be opened from the link at the bottom of this page.
We will then process your request, and once ready, you will be able to download copies of the records from the portal.
We accept requests for copies of health & care records from:
Patients/service users/clients requesting copies of their own records or people acting on their behalf.
Individuals or organisations (e.g., solicitors, social care providers) requesting copies of records, reports, or letters
Coroners, police, or by court order
People such as executers of wills accessing the records of the deceased
