Occasionally, problems occur while we are looking after you which affect your care. We call these problems incidents. Incidents can vary from very small problems, such as a delay in being seen for an appointment, to incidents that result in harm.
Patient Safety Incident Response Framework
The Your Healthcare Patient Safety Incident Response Framework (PSIRF) sets out a new NHS approach to responding to patient safety incidents for the purpose of learning and improving patient safety. To view the plan please click here.
