If you are a professional working for another organisation and have a concern about any aspect of the quality of care being provided by us, you should complete our Quality Concern form.

You will receive an acknowledgement and your concerns will be investigated in line with the Your Healthcare Quality Concern Process.

Please note that this process is for quality concerns that fall below the threshold for safeguarding. If you have raised a safeguard alert, then you do not need to complete a quality concern form.
