The Cardiac Rehabilitation team cares for people with heart disease. Our aim is to empower people to self-manage their condition and recover from a serious cardiac event such as heart attack or unstable angina.
We offer a cardiovascular disease prevention and rehabilitation programme which is designed to help you recover from a heart problem, improve your physical health and your quality of life. All programmes offer:
- Cardiovascular exercise
- Education
- Risk factor management
- Stress management and relaxation
- Support
What we do
All our cardiac rehabilitation programmes start with an individual assessment, either face-to-face, by phone or video consultation, and we will work with you to set some goals and choose from the programmes available. During your programme we continue to offer support.
Personalised care
We develop a tailored rehab plan offering cardiac exercise, education and relaxation training. We offer telephone and video calls, web-based programmes, exercise videos, a walking programme, education and support via Microsoft Teams and home visits where needed. Typically, people spend 2-3 months on their rehab programme. When the programme has finished we liaise with other healthcare providers, such as GPs, about ongoing care and needs.

We offer a 6-week online education programme via Zoom covering:
- Heart disease and treatments
- Medication for the heart
- Risk factors for CHD and how to make successful lifestyle changes
- Healthy eating
- Exercise for a healthy heart
- Psychological effects of heart disease
Exercise Programme
You may enjoy exercising with others, if so this 6-week programme may be for you. During the programme you will be challenged to do more weekly exercise. Whichever programme you choose we are here to support you. We will contact you at regular intervals and you can contact us as needed.
The Heart Manual
The Heart Manual is a 6-week programme for individuals recovering from myocardial infarction (Heart Attack or MI) or revascularisation (angioplasty, stents or bypass graft surgery). You will be given a book or an online login to a website with a wealth of information to help you.

We are an independent community interest company (CIC), but are funded by the NHS and local authorities to provide health and social care community services for residents in Kingston, and additional learning disability services in Richmond. If we make any profit, it is reinvested into our community services

You can also call for emergency referrals to our Rapid Response service (2-hour urgent community response) or Urgent Care Support Service (care homes).

You can also call our single point of access team on 020 8274 7088 for referrals to our Rapid Response service (2-hour urgent community response).
