Tissue Viability

Tissue Viability is a nurse-led service that specialises in all complex wound management including pressure ulcers, leg ulcers, skin tears, surgical wounds, malignant wounds and certain skin-related concerns.

The service provides specialist advice, assessment, treatment and support to people registered with a Kingston GP.

We work closely with local health care professionals (HCPs) including community nurses, practice nurses, care home staff, and other services such as hospital teams and social care services. Our overall goal is to ensure that any patient who has an active wound or is at risk of developing wounds, receives the best possible care to enable effective healing and prevent the problem happening again.

The service also provides education, training and support to HCPs and higher education institutions within the Kingston Borough.

Our Service

Our community tissue viability nurses provide advice, support and treatment to GP practices and directly to patients who are housebound, in care homes or on community inpatient wards.

How to access our Service

Referrals are currently accepted from health care professionals (HCPs) only.

If you are an HCP within South West London and require our support, please refer or contact us via our Single Point of Access: please click on the referral form in the ‘Looking to make a referral’ box below.

If you are a patient and have a problem with a non-healing wound, lower limb swelling or a general problem with skin health (also called skin integrity), please contact your HCP to discuss a referral.

We are an independent community interest company (CIC), but are funded by the NHS and local authorities to provide health and social care community services for residents in Kingston, and additional learning disability services in Richmond. If we make any profit, it is reinvested into our community services

You can also call for emergency referrals to our Rapid Response service (2-hour urgent community response) or Urgent Care Support Service (care homes).

It is possible to self refer to some of our services (e.g. rapid response, children’s speech and language). If you click on the ‘Our services’ tab and choose the service you need, details of how to refer are available. You can also call our single point of access team on 020 8274 7088 for referrals to our Rapid Response service (2-hour urgent community response).
