Community Nursing

Community nursing teams provide skilled nursing care to adults in their own homes, in residential homes or in a clinic setting. Patients are primarily housebound and will attend their GP’s surgery as they recover.

We are attached to GP surgeries, working as part of an integrated team. We work closely with families, carers and other professionals including those in other healthcare settings, private and voluntary sectors. We also provide care to patients requiring palliative and end-of-life care.

Community nursing teams are led by district nurses – specialist practitioners in this area of work. Community matrons are also key members of the team.

Supporting independence

We support and encourage people with a disability or long-term condition to live independent lives, providing a range of services and treatment to enable them to avoid unnecessary hospital admissions and to support discharge as soon as it is appropriate, when a hospital admission has been necessary.

To contact Community Nursing:

Between 8am and 6pm Monday to Friday and 9am to 5pm Saturday and Sunday/Bank Holidays, contact the Single Point of Access on 020 8274 7088.

Outside these hours contact via 111 who will contact the community nursing services.

 

Overnight service

We work in partnership with the 8 to 8 Night Response Service to ensure that support is available to patients overnight. The service provides urgent nursing care to prevent hospital admission if appropriate, and can be contacted via 111.

Your Healthcare also works in partnership with Princess Alice Hospice, which provides palliative and end of life care to patients, supporting them with advice and symptom control.

We are an independent community interest company (CIC), but are funded by the NHS and local authorities to provide health and social care community services for residents in Kingston, and additional learning disability services in Richmond. If we make any profit, it is reinvested into our community services

You can also call for emergency referrals to our Rapid Response service (2-hour urgent community response) or Urgent Care Support Service (care homes).

It is possible to self refer to some of our services (e.g. rapid response, children’s speech and language). If you click on the ‘Our services’ tab and choose the service you need, details of how to refer are available. You can also call our single point of access team on 020 8274 7088 for referrals to our Rapid Response service (2-hour urgent community response).
