Community Matrons

Expert care for patients

Community matrons provide expert care for patients with one or more long-term conditions who are at risk of deteriorating so that a hospital admission would be necessary, or their quality of life would be affected significantly.

They are integrated within the wider district nursing teams and work closely and in partnership with other services including the rapid response team, urgent care services, specialist nurses, end-of-life lead and the reablement team, to provide joined up holistic care to patients and their families.

Who do we support?

Community matrons are highly skilled, experienced, senior nurses. They work with patients who have a wide range of conditions, often serious long-term conditions or a complex range of conditions. These include diabetes, Chronic Obstructive Pulmonary Disease (COPD), Coronary Heart Disease, Parkinson’s disease, Multiple Sclerosis and HIV.

Keeping people out of hospital

By caring for people in the community, community matrons can prevent unnecessary hospital admissions, reduce the length of time people stay in hospital if an admission is needed, improve the quality of life for patients and support them to manage their own conditions.

Community Nursing

Community nursing teams provide skilled nursing care to adults in their own homes, in residential homes or in a clinic setting. Patients are primarily housebound and will attend their GP’s surgery as they recover.
