Community Neuro Rehabilitation
Multidisciplinary rehabilitation support
Your Healthcare provides specialist, multidisciplinary rehabilitation support to adults with a neurological diagnosis who are aged over 18 years and registered with a Kingston GP.
The most common neurological conditions include:
- Stroke
- Multiple Sclerosis
- Motor Neurone Disease
- Parkinson’s Disease
- Brain Injury
We support and empower individuals and their carers to self-manage their conditions to help maximise their independence and have a better quality of life.

Where we work
Our team works in a variety of settings wherever it is most appropriate. This can be in someone’s own home, at an outpatient clinic, or in any other appropriate community setting including gyms, day centres, places of work.
The core team comprises:
Speech and Language Therapy; Occupational Therapy; Physiotherapy; Dietetics.
We can see people from 16 years of age on a case-by-case basis, please contact the team to discuss these referrals.
Our approach
We offer comprehensive, specialist assessments, treatment, advice and education on issues relating to neurological conditions including:
- Pain
- Cognitive impairment
- Mobility
- Daily living activities
- Communication
- Swallowing
- Food and nutrition
- Returning to work
- Management of long-term conditions
- Early supported discharge for stroke
- Six-month stroke reviews

Referrals are accepted via fax, letter, or telephone from health and social care professionals through our Single Point of Access:

Please include details of the presenting problem and diagnosis (if known), identified functional goals and all relevant contact information.
We work closely with each person to provide a goal-orientated approach and rehabilitation package that meets individual needs, with an emphasis on achieving independence with daily tasks. Treatment can be provided on a 1:1 basis or within a group, depending on need.

We are an independent community interest company (CIC), but are funded by the NHS and local authorities to provide health and social care community services for residents in Kingston, and additional learning disability services in Richmond. If we make any profit, it is reinvested into our community services

You can also call for emergency referrals to our Rapid Response service (2-hour urgent community response) or Urgent Care Support Service (care homes).
It is possible to self refer to some of our services (e.g. rapid response, children’s speech and language). You can also call our single point of access team on 020 8274 7088 for referrals to our Rapid Response service (2-hour urgent community response).
