Dementia
Supporting your needs
The Dementia Service at Your Healthcare is led by the frailty, dementia and palliative care specialist nurse. Dementia support is delivered by a specialist dementia practitioner, who provides information, health education, assessment of dementia progression, advice and ongoing support to individuals with a formal diagnosis of dementia.
Patients will:
- have a confirmed diagnosis of dementia
- be registered with a Kingston GP
- no longer be under the care of the memory clinic or SW London mental health team for older people,
- have clinical as well as social care needs
People who only have social care needs should be referred to Kingston Adult Social Care.
Our aims
We work closely with colleagues in health and social care and with voluntary organisations to deliver an integrated service.
We aim to:
- deliver person-centred care
- share information with families and caregivers to better prepare them for their caring role
- reduce hospital admissions for people living with dementia.
Accessing our service
To access the dementia service, patients must be registered with a Kingston GP and have a formal diagnosis of dementia. We accept referrals from a wide range of sources, including:
- General Practice
- Voluntary sector organisations
- Alzheimer’s Society
- Kingston Hospital
- Adult Social Care
- Self-referrals
Please note: This service does not provide initial diagnostic assessments for dementia. Individuals seeking a diagnosis should speak to their GP for referral to the memory clinic.

We are an independent community interest company (CIC), but are funded by the NHS and local authorities to provide health and social care community services for residents in Kingston, and additional learning disability services in Richmond. If we make any profit, it is reinvested into our community services

You can also call for emergency referrals to our Rapid Response service (2-hour urgent community response) or Urgent Care Support Service (care homes).
It is possible to self refer to some of our services (e.g. rapid response, children’s speech and language). If you click on the ‘Our services’ tab and choose the service you need, details of how to refer are available. You can also call our single point of access team on 020 8274 7088 for referrals to our Rapid Response service (2-hour urgent community response).
