Diabetes Care

Your Healthcare provides support, advice and education for people with diabetes living in the Kingston area.

The service is led by a community diabetes specialist nurse whose main focus is the DESMOND programme, but she may visit patients at home or in a GP surgery (mainly to support GPs and community or district nurses).

DESMOND is a nationwide group education programme developed by experienced health care professionals and people who have diabetes.

The programme is for those newly diagnosed with Type 2 Diabetes and there is also Foundation DESMOND for those with established diabetes.

DESMOND Programme

DESMOND is delivered by healthcare professionals who are specially trained to deliver the course. Its aim is to support the person with diabetes, to become an expert in his/her condition.

Patients receive six hours of structured education in a group setting with a family member or friend.

We are an independent community interest company (CIC), but are funded by the NHS and local authorities to provide health and social care community services for residents in Kingston, and additional learning disability services in Richmond. If we make any profit, it is reinvested into our community services

You can also call for emergency referrals to our Rapid Response service (2-hour urgent community response) or Urgent Care Support Service (care homes).

It is possible to self refer to some of our services (e.g. rapid response, children’s speech and language). If you click on the ‘Our services’ tab and choose the service you need, details of how to refer are available. You can also call our single point of access team on 020 8274 7088 for referrals to our Rapid Response service (2-hour urgent community response).

Only urgent services can be referred via telephone – Rapid Response and Urgent Care and Support Service (care homes).
