Family Support Service: before and after diagnosis
Guiding families to the best support
The SEND (special educational needs and disabilities) Family Support Team can help guide families to the best support available, beginning before a diagnosis has been made and continuing until children reach adulthood. This specialist support service consists of a team lead, nurse/health visitor and community nursery nurse, covering both Kingston and Richmond. They work within a wider team including occupational therapists, physiotherapists, and speech and language therapists.
Families with children aged 0 to 19 years who need advice, support or information will be invited to drop in sessions, coffee mornings and workshops covering topics such as: introduction to autism; behaviour that challenges; restrictive eating; sleep and toileting.
Who is the service for?
Referrals will be considered for children and young people who have been given a confirmed or suspected neurodevelopmental diagnosis and need specialist support. They are likely to attend either The Moor Lane Centre, Chessington or Ham clinic. Referrals can be made by community paediatricians, other professional, or parents and carers.

Referrals are likely to be for children who have been given a diagnosis/ suspected diagnosis of a neurodevelopmental disability/disorder, autism or social and communication difficulties, or who have had a referral accepted and are waiting for further assessment
Referrals 5-19 years
Referrals are likely to be for children who:
- have been given a diagnosis/suspected diagnosis of a neurodevelopmental disability/disorder
- have been referred for an autism/neurodevelopment disability assessment
- have had an assessment with CAMHS (Children and Adolescent Mental Health Services) and are waiting for an assessment for social and communication difficulties, or autism.

We are an independent community interest company (CIC), but are funded by the NHS and local authorities to provide health and social care community services for residents in Kingston, and additional learning disability services in Richmond. If we make any profit, it is reinvested into our community services

You can also call for emergency referrals to our Rapid Response service (2-hour urgent community response) or Urgent Care Support Service (care homes).

You can also call our single point of access team on 020 8274 7088 for referrals to our Rapid Response service (2-hour urgent community response).
