Your Healthcare’s palliative care nurses play a crucial role in enhancing the quality of life for patients with serious illnesses. Their compassionate care and specialist expertise provide comfort, symptom management and emotional support to those in their final days of life. They also provide vital support for families and carers.
Palliative care aims to alleviate pain, manage symptoms and address the physical, emotional and psychological needs of patients, as well as families. Unlike curative treatments, palliative care aims to enhance comfort and overall well-being, emphasising open communication and shared decision-making between patients, their families and healthcare providers.

Between 8am and 6pm Monday to Friday and 9am to 5pm Saturday and Sunday/Bank Holidays, contact the Single Point of Access on 020 8274 7088.

Community Nursing
Community nursing teams provide care to adults in their own homes, in residential homes or in a clinic setting, primarily for housebound patients who will attend their GP surgery as they recover. The teams are led by district nurses – specialist practitioners in this area of work. Community matrons are also key members of the team.

The urgent care and support team assesses, diagnoses, prescribes and treats acute health problems, to prevent hospital admissions for patients living in care homes. Our team of paramedics and nurses offer 2-hour clinical triage and same day visits

We are an independent community interest company (CIC), but are funded by the NHS and local authorities to provide health and social care community services for residents in Kingston, and additional learning disability services in Richmond. If we make any profit, it is reinvested into our community services

You can also call for emergency referrals to our Rapid Response service (2-hour urgent community response) or Urgent Care Support Service (care homes).
